Person-Centred

Working in a person-centred manner means focusing on the patient as a unique individual with their own needs, values, and preferences. The team considers the patient's perspective, listens to their concerns, and involves them in health-related decisions.

Holistic

This approach is holistic, taking into account all aspects of a patient's health including physical, mental, social, and spiritual well-being. Healthcare professionals assess not only symptoms and illness but also the patient's life situation, lifestyle, and social support systems. The goal is to provide comprehensive healthcare services that meet the patient's complex needs.

Proactive

Working proactively involves acting preventively and predictably to avoid problems before they occur or worsen. The patient's team initiates efforts to identify risk factors, implements preventive measures, and collaborates with the patient to maintain good health. The aim is to prevent unnecessary exacerbations of illness and enhance the patient's quality of life.

When combined in a patient's care journey, this approach signifies care and health services centred around the patient's needs, considering their entire life situation, and attempting to prevent issues before they arise. This can enhance patient involvement, improve treatment outcomes, and promote public health.
